Effective January 1, 2010 everyone will be required
to use the attached Annual Health and Medical Record. The National office
has advised that the existing stock of Class 1, 2 and 3 forms continued to
be used while our supply lasts. Once our supply is gone we will convert to
the new form.
The Annual Health and Medical record is available
online at www.scouting.org under "Scouting
Safely". It is posted in a downloadable PDF format.
Attached is a Frequently
Asked Questions sheet that
should answer most of your questions regarding the new Helth and Medical Record
form. If you have any further questions, please call the Scout Service
Center.
